Be part of our PACE (Program of All Inclusive Care for the Elderly) Program with sites in West Palm Beach and Delray Beach.  PACE is a managed care plan for seniors age 55 and older who are nursing home-eligible, but prefer to remain at home with healthcare and related services.

PACE Physical Therapist

In this role, you will be responsible for the day-to-day operations and coordination of PACE services that are delivered to participants. This includes participation with the Interdisciplinary Group and oversight of the programs operations and systems. 

 

How to make an impact:

  • Provides physical therapy evaluation, treatment, and interventions to program participants.

  • Conducts initial assessments and reassessments, upon referral from team, of participant’s condition and functional status and establishes measurable goals and a treatment plan.

  • Provides training, guidance, and supervision to program staff and participants/family members regarding therapy.

  • Responsible for providing and recommending therapeutic exercises.

  • Participates actively in the interdisciplinary group by providing clinical expertise in team planning, problem solving, and processes.

  • Fulfills all administrative requirements, including but not limited to collaboration with Occupational Therapist, attendance at team and staff meetings, and recording and tracking of physical therapy durable medical equipment.

  • Develop treatment program when indicated, sets goals and provides treatment.

 

Job Requirements

 What you’ll bring to the table:

  • Be a graduate from a physical therapy curriculum approved by one of the following:

    • The American Physical Therapy Association

    • The Committee on Allied Health Education & Accreditation of the American Medical Association

    • The Council on Medical Education of the American Medical Association and the American Physical Therapy Association

  • Clear / Active FL PT License

  • Valid driver’s license and automobile insurance in the State of Florida.

  • 3- 5 years experience with a minimum of one year experience with the frail and elderly.

  • Reliable transportation to meet visit schedule.

  • Ability to use equipment with visual and auditory mechanisms.

  • Ability to perform the essential functions and physical requirements (including, but not limited to: lifting participants and/or equipment, bending, pushing/pulling, kneeling) of the job with or without reasonable accommodation.

  • At least one year of experience working with the frail elderly.
