Physical Therapist Assistant (PTA)

In this role, you will implement an individual patient therapy care plan that has been prepared by the Physical Therapist and work under the supervision of the Physical Therapist. You will also report to the Supervisor, Rehab Services


How to make an impact:

  • Perform routine strengthening exercises and therapeutic exercises as outlined by the lead PT
  • Assist in teaching exercise programs to patient and family
  • Consult with therapist regarding the outcome of home visits and report findings in written forms
  • Submit written daily notes to Clinical Office within 72 hours of visit
  • Observe and record activities and findings in the clinical record
  • Instruct other health team personnel including HHAs and family members in certain aspects of physical therapy with which they work with the client
  • Participate with staff, client and physician in discharge activities and assists the PTA in completing the discharge summary
  • Communicate effectively with all those providing care

Job Requirements

What you’ll bring to the table:

  • Associate's degree or equivalent from two-year College or technical school
  • Must have at least 2 years' experience
  • Active CPR license
  • Experience in a Home Health environment
  • Must have reliable transportation, current driver's license, and proof of current auto insurance.
  • Excellent organizational and communication skills.
