PACE – Transportation Coordinator

In this role, you will be Responsible for safe transport of Clients.

How to make an impact:

  • Use Nextraq for scheduling purpose

 

  •  Schedule the PACE clients for their visits to daycare and Doctor’s office

 

  • Schedule the Drivers for their daily routes

 

  •  Prepare the schedule and dispatch it to the P.N.D., which is mounted on the Buses and then print paper copies of the schedules for the Drivers

 

  • Check e-mail and phone for new notifications

 

  •  Carry out the Transportation/ Medication drop off request entered in the PACE Logic

 

  •  Enter the Transportation data into the transportation log daily

 

  • Attend IDT for transportation input/care plans

 

  • Communicate with members of IDT staff including clinic staff & center manger regarding status updates, delivers, etc.

Job Requirements

What you’ll bring to the table:

  • High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.
  • A valid driver's license, automobile insurance and reliable transportation.
  • Minimum of 1-year experience as a driver and must possess a valid Commercial Drivers License (CDL), Class A, B or C certification with Passenger (-P) or School Bus (-S) Endorsement.
  • Minimum of 1-year experience with the frail and/or elderly
  • CPR Certification
