Social Worker 

The Social Worker is a professional with specialized skills in the psychological, emotional and
social aspects of geriatric client care, with a working knowledge of community and social
resources. He/she should have a sensitive understanding of the special needs of clients with
chronic, treatable illnesses and the elder population. He/she provides direct care management
services as appropriate to individuals and families receiving MLCMC. He/she should have a
sensitive understanding of the special needs of caregivers and care recipients who have chronic
illness and the elder population.

 

How to make an impact:

  • Manages a caseload of ten to fifteen clients who have chronic illnesses and need assistance in obtaining community resources and addressing psychosocial issues. Caseload consists of caregivers needing psychological support.
  • Completes comprehensive assessments on a timely basis as per the admissions and readmissions process.
  • Assesses the social and emotional factors related to the client chronic illnesses/psychological issues. Coordinates plan and provides counseling and support to the client and family based on an assessment.
  • Completes treatment plan with clients and families to assist with interaction of client.
  • Coordinates individual/family counseling and group support to assist the client/family to cope with the care recipient’s illness.
  • Complies with grant related activities and mandates, as well as HIPPA and MLCMC Policies.
  • Identifies and intervenes with suicidal individuals. Assesses for substance abuse, abuse or neglect.
  • Assesses financial situation of clients as appropriate, interpreting the medical situation and referring clients/families to community agencies as required.
  • Identifies environmental safety hazards and risk factors (emotional suicidal individuals) and educates the client/family accordingly.
  • Completes all required documentation in a timely and accurate manner.
  • Provides community outreach and works with Clinical Director to build case management census.
  • Assesses issues with cultural diversity.
  • Attends all mandatory meetings.
     

Job Requirements

What you’ll bring to the table:

  • Master's degree (M. A.) or equivalent; or four to ten years related experience and/or training; or equivalent combination of education and experience.
