Be part of our PACE (Program of All Inclusive Care for the Elderly) Program with sites in West Palm Beach and Delray Beach.  PACE is a managed care plan for seniors age 55 and older who are nursing home-eligible, but prefer to remain at home with healthcare and related services.

PACE – Registered Nurse

Provides nursing care is in accordance with the patient’s plan of care, including: comprehensive assessment, monitoring of the health status, coordination of services, teaching and training activities, and management of the care plan. **MONDAY THROUGH FRIDAY ONLY.**

 

How to make an impact:     

Conducts the admission of the patient to PACE including:

o Providing initial and ongoing comprehensive assessment of the client’s needs

o Obtaining a medical history, particularly as it relates to the present condition, for the patient and/or family member(s)

o Conducting a physical examination of the patient, including vital signs, physical assessment, mental status, appetite and type of diet, etc.

o Evaluating the patient, family member(s) and home situation to determine what health teaching will be required

o Evaluating the patient's condition and home situation to determine if the services of a home health aide will be required and the frequency of this service

o Interpreting nursing and other services of the Agency to patients and families as a part of planning for care. Developing and implementing the plan of care.

o Conducts initial and ongoing physical, psycho-social functional and nutritional assessments appropriate to patient service status

Providing skilled nursing care as outlined in the plan of care, including:

o Nursing services, treatments and preventative procedures requiring substantial specialized skill and ordered by the physician

o Initiating preventative and rehabilitative nursing procedures as appropriate for the patient's care and safety

o Observing signs and symptoms and reporting to the physician reactions to treatments, including drugs, and changes in the patient's physical or emotional condition

o Teaching, supervising and counseling the patient and caregivers regarding the nursing care needs and other related problems of the patient at home.

o Administers prescribed medication

o Participates in patient/family teaching activities

Responsible for the care given by the PACE clinical staff by including supervision of the caregiver and the home health aide plan of care

o Consults with the attending physician or ARNP for changes in the plan of care in consultation with the supervisor. Obtains complete orders for care

o Submits clinical notes no less often than weekly, and progress notes and other clinical record forms outlining the services rendered as indicated

o Participates in the development and periodic revision of the physician's Plan of Care and processes change orders as needed. Participates in the patient’s discharge planning process

o Participates in Quality Improvement Activities

o Spends time in office doing chart reviews

Must travel between all PACE centers and participant’s homes.

 

Job Requirements

What you’ll bring to the table:

 

(EDUCATION and/or EXPERIENCE)

  • Registered Nurse Degree. Bachelor’s Degree preferred.
  • 6mo. to 1 year of progressive experience in clinical positions.
  • At least one year experience working with the frail elderly is preferred

  • Knowledge of regulatory standards and compliance requirements.
  • Ability to use communication problem-solving and management skills to influence staff utilization and motivation, public relations, and inter and intra departmental relations.
  • Demonstrates a caring attitude toward PACE participants, family members, employees and other facility guests.
  • Ability to supervise LPNs and Home Health Aides as needed

 

CERTIFICATES, LICENSES, REGISTRATIONS

  • Current RN License in the State of Florida
  • BLS
  • ANCC or specialty certification preferred.
  • CPR Certification
