LPN/RN MDS Coordinator

Coordinates the MDS process; audits and produces plan of correction, and manages sign off process for assigned areas.

How to make an impact:

  • Conducts and coordinates care plan meetings as needed
  • Functions as an integral part of the Quality Improvement Program for care planning, MDS
  • Meets resident/patient and gathers information on all new admissions, re-admissions, and transfers per assigned units
  • Assesses resident/patient per RAI guidelines

 

  • Follows all OBRA and PPS guidelines as it applies to the RAI process
  • Audits and Reviews resident/patient chart for documentation; discrepancies, quality, reimbursement requirements, and risk for liabilities
  • Implements plan of correction and provides education to resolve audit findings; updates interdisciplinary team and MDS Manager regarding corrections as necessary
  • Process QM audits as well as regular MDS audits for State and MDS survey
  • Review support documentation for key clinical areas including; wound care, falls, weight loss, dialysis, foley catheter, and significant change
  • Provide Nurse Manager support according to coverage rotation schedule
  • Assist with dining on units as scheduled
  • Participate in administrative rounds as assigned
  • Serves as unit champion
  • Serves as MDS manager back-up
  • Other duties as assigned

Job Requirements

What you’ll bring to the table:

  • Associate’s degree or equivalent from two-year College or technical school and a minimum of 2 years’ experience in the field.
  • RAC-CT certification required
  • Must have prior experience with MDS and care plans.
  • Valid FL LPN/RN license and CPR (LPN required RN preferred)
  • Must have prior experience with MDS and care plans.
