Occupational Therapist (OT)

In this role, you will be trusted to provide skilled occupational therapy services, on an intermittent or as-needed basis to patients in their homes.

 

How to make an impact:

  • Assist in development and implementation of the interdisciplinary client care plan as it pertains to Occupational Therapy in consultation with the physician.

  • Provides skilled Occupational Therapy as outlined below and after consultation with physician.

  • Evaluates the client's level of function by applying diagnostic and prognostic procedures.
  • Guides and instructs the client in prescribed therapeutic self-care and creative activities that are directed toward improving independence and physical and mental functioning.
  • Instructs client in care and use of wheelchairs, braces, splints, and prosthetic/orthotic devices.
  • Teaches and supervises and counsels the family and client in the total occupational therapy program and other related problems of the client at home.
  • Establishes goals based on the client's current level of functioning and potential for improvement and performs re-evaluations as indicated.
  • Evaluates the effectiveness of therapy intervention and updates the client care plan as needed.
  • Communicates with the physician on a regular basis regarding client's status, obtaining additional orders, if necessary.
  • Prepares clinical/progress notes on the day of each visit:
  • Communicates with other team members and, when appropriate, instructs them in occupational therapy techniques, as appropriate.
  • Participates in agency activities, in-services, meetings, when appropriate.
  • Supervises the Occupational Therapy Assistant at least monthly.
  • Participates with staff, client, and physician in discharge activities and completes discharge summary.

Job Requirements

 What you’ll bring to the table:

  • An Associate’s degree or equivalent from a two-year college or technical school, and 2 years of Occupational Therapy experience (or equivalent combination of education and experience)
  • Certification as a Registered Occupational Therapist in the State of Florida
  • Active BLS CPR license.

  • 1  year Experience in a Home Health environment.

  • Must have reliable transportation, current driver's license, and proof of current auto insurance.

  • Excellent organizational and communication skills.
