Be part of our PACE (Program of All Inclusive Care for the Elderly) Program with sites in West Palm Beach and Delray Beach.  PACE is a managed care plan for seniors age 55 and older who are nursing home-eligible, but prefer to remain at home with healthcare and related services.

PACE Registered Dietitian

In this role, you will be responsible for the day-to-day operations and coordination of PACE services that are delivered to participants. This includes participation with the Interdisciplinary Group and oversight of the programs operations and systems. 

 

How to make an impact:

  • Perform nutritional assessments upon enrollment and re-assess every 6 months or as needed for significant changes in clinical status. Maintain food preferences and tray tickets for designated participants.   Record periodic nutritional care data in the medical record. Review and adjust therapeutic diets to reflect any changes in the daily menu and suggest the use of supplemental feedings when necessary. Provide diet instruction to participants and/or family members on prescribed diets.

     

  • Assist in providing administrative direction for meal service; establish programs, policies and procedures that will enforce safe food handling practices.

     

  • Coordinates both inter and intra departmental professional activities to provide optimum nutritional care for the participants and serve as consultant to clinical staff on matters pertaining to dietetics and nutrition. Attend and participate in Interdisciplanary Team (IDT) Meetings as designated. 

     

  • Assist in development and delivery of departmental and facility-wide educational programs.

     

  • Ensure that all clinical issues meet and/or exceed Federal, State and County regulations.

     

  • Maintain rapport with participants, visitors, and family members and provide feedback on the overall PACE participant attitude about food service.

     

  • Provide Medical Nutrition Therapy (MNT) to participants as ordered by medical provider.

Job Requirements

What you’ll bring to the table:

  • B.S.  in Nutrition.

  • Able to teach, think analytically and relate to geriatric patients.

  • Knowledge of nutrition care process and federal, state and local standards and regulations.

  • Licensed in the state of Florida

  • 1-3 years of experience. Must have at least one year experience working with the frail and elderly.

  • Valid driver’s license and automobile insurance in the State of Florida.

  • Ability to effectively communicate in English.

  • Ability to perform the essential functions and physical requirements (including, but not limited to: lifting participants and/or equipment, bending, pushing/pulling, kneeling) of the job with or without reasonable accommodation.
