PACE – Physical Therapy Assistant

In this role, you will implement an individual patient therapy care plan that has been prepared by the Physical Therapist and work under the supervision of the Physical Therapist. Reporting to the Supervisor, Rehab Services.

How to make an impact: 

  • Perform routine strengthening exercises and therapeutic exercises as outlined by the lead PT
  • Assist in teaching exercise programs to patient and family
  • Consult with therapist regarding the outcome of home visits and report findings in written forms
  • Submit written daily notes to Clinical Office within 72 hours of visit
  • Observe and record activities and findings in the clinical record
  • Instruct other health team personnel including HHAs and family members in certain aspects of physical therapy with which they work with the client
  • Participate with staff, client and physician in discharge activities and assists the PTA in completing the discharge summary
  • Communicate effectively with all those providing care

Job Requirements

What you’ll bring to the table:

  • Current professional licensure
  • Associate's degree or equivalent from two-year College or technical school
  • 2 years' experience preferred
  • 1-3 years experience with a minimum of 1 year experience working with the frail and elderly
  • Ability to perform physical requirements of the position, including being able to lift (with lift devices or assistance) residents in excess of 100 pounds.
  • Developed written and verbal communication skills.
  • Effective multi-tasking and problem solving skills.
  • CPR Certification
  • Valid Driver’s License and automobile insurance
